MTA & Tournament Organizers will verify each and every registered player. Players under the age of 16 will need parent or guardian approval to compete. MTA Organizers will call each guardian for verbal authorization in addition to this written release.

All players must submit a completed form by the start of the tournament (September 13th) to be eligible for prizing and awards. Every player must complete and submit the form themselves. Registration completed by one person for multiple people will NOT be accepted.
Players can compete in multiple games should they choose.
